NHS@Home. In Bristol, North Somerset, and South Gloucestershire (BNSSG), we are working to ensure health and care services make sense to the people using them. NHS@Home is a new model of care that brings together health and social care providers from Acute Trusts and Community partners to deliver joined up care from the comfort of people’s own homes, supported by remote technology and remote health monitoring where appropriate.
Working as part of Healthier Together, UHBW NHS Trust, NBT NHS Trust and Sirona care & health are collaborating to bring acute care to people in the comfort of their own homes.
‘NHS@Home’ aspires to help people to stay well and at home by facilitating self and supported monitoring of:
- Chronic conditions in the community, aiming to reduce the need for admission to hospital
- Acute illness, aiming to ensure as little time as possible is spent in an acute hospital bed
We know that people recover faster and better in their own environments and NHS@Home endeavours to support this.
For this exciting new venture, we are looking for enthusiastic people to join the team, either short term or long term posts, with hours to suit. We are recruiting people who can communicate and connect well with others, help problem-solve and are interested in being part of an innovative and supportive team, leading change to the way we provide healthcare.
We have diverse opportunities both for people with and without clinical skills, IT and tech skills or training. All backgrounds are welcome and no skill set is too small.

You may also want to volunteer, perhaps as a driver, to drop off medications, or being a ‘digital friend’ to give people a call or check kit and equipment. The NHS in partnership with the Royal Voluntary Service are looking for an ‘army’ of volunteers who can support the people in England who have long term health conditions (and/or are at most risk from covid) to stay well.
